The effort to reach everyone in our communities with the Advance Care Planning message is dependent, in large part, on partnerships.
Our Advance Care Planning initiative benefits from two types of partnerships: curriculum partners and host/venue partners.
- Curriculum partners are typically healthcare organizations that use already established curriculum or their own in-house developed curriculum to reach people with the advance care planning message. These partners typically schedule, promote and host their own classes and sessions. The Snohomish County Health Leadership Coalition also promotes these classes to the community. - Host/venue partners can be any organization (non-profit, business, church, agency, etc.) willing to host an advance care planning class for their community members or the general community. The curriculum presented is usually the Respecting Choices® curriculum by Gundersen Health System. Classes are taught by trained facilitators either within or outside the host/venue partner organization and materials are provided by the Advance Care Planning initiative team.
