Advance Care Planning

We plan for many events in our lives, from weddings to buying a home, our careers and, of course, retirement. Have you created your plan in case you have a medical emergency or a serious illness? Would your family know your wishes if you were unable to speak for yourself?

Advance Care Planning is the process of making decisions – based on your personal values and preferences – about the kind of care you want to receive if you can’t speak for yourself. It’s not just for those who are critically ill or elderly. It’s for everyone. Right now. Just in case.
